I have both Sneaky Pix and SecretCam. I hardly use them, but sometimes I want to take a shot surreptitiously (say without my son knowing I am taking it), and they come in handy that way. It also can legitimately be used to document a moment without making the subject nervous with the notion that the camera is on them.
For instance, I often do social protest photography. Frequently, these are large public events, but at times I have spooked folks with large dslr gear. Walking closer to a crowd, and using your iPhone surreptitiously might not spook the people, and does not lead to a change in behavior as they become hyper-aware of the camera gear.
This has its challenges (angle, shooting blind, etc.) but it can indeed work out well also.

DG Cam is the best of the stealth camera apps that I have tried.

StealthCam
I saw this on Apptism in the previews today - it's not out yet but I can imagine it'll be pretty popular with the people of walmart and look at this f*cking hipster crowd, ha. I've definitely been in situations where I wanted to take a picture of something but didn't want people to know - nothing sketchy but stuff I was embarassed to admit I was photographing.
But I imagine with all the genuinely sketchy people out there this app will bring up some ethical issues...
